Referral Information and Forms
All staff members (clinicians, front desk staff, volunteers, and trainees) are bilingual in Spanish and English. All signs posted in our clinics, as well as patient education materials, are either in Spanish or bilingual.
We accept Medicaid, Health Choice, and other insurances. We also serve those who do not have insurance. For further questions surrounding payment, we encourage clients to attend our orientation appointments. El Futuro firmly believes that money should be no obstacle in accessing quality healthcare.
We are currently accepting new clients seeking behavioral health services in Durham and Siler City!

There will be walk-in clinic hours starting on February 18th, 2015. These will be for new intakes only. If you have insurance, please make sure to bring your insurance information with you when you arrive to the clinic. If this is a JCPC referral, please have the family bring the JCPC referral form to the first appointment.
Starting February 18th, clinics will take place every Wednesday and Thursday, from 9-11 am, and 12-2 pm.
On March 3rd, we will add a Tuesday clinic from 9-11 am, and 12-2 pm.
From March 9th onwards, clinics will take place every Monday through Thursday, during 9-11 am and 12-2 pm each day.
